For professionals

FaceFormer therapy trainingSeminars and online courses

For dentistry, orthodontics, speech and language therapy, physiotherapy, osteopathy and myofunctional therapy. Two days with Dr. Klaus-Jürgen Berndsen and Sabine Berndsen: understand the cranio-cervical functional model, assess breathing, swallowing and head balance, and practise the exercises yourself.

The key points at a glance

  • The two-day training shows you how breathing, swallowing and head balance work together, how to assess deviations and how to guide FaceFormer therapy in your practice.
  • Day one is devoted to understanding and diagnosis, day two to application. You practise yourself, discuss your own cases and receive a certificate as well as a certificate of attendance from ISST Unna with CPD points.
  • The last in-person seminar took place on 25 and 26 September 2026 in Frankfurt am Main. New dates have not yet been set, but you can already put your name down.

The seminar fee included a FaceFormer training set, the manual on the methodology and catering at the conference hotel. You do not need any prior knowledge. The seminar is aimed at newcomers as well as professionals who already work with the FaceFormer and want to structure their approach.

Target group

Who the training is for

The method addresses functions that play a role in many specialist fields. That is why professionals from very different disciplines sit at the same table in the seminar.

Dentistry

Classify craniomandibular dysfunction (TMD), bruxism and tooth migration from a functional perspective. You will learn which findings point to a disturbed interplay of breathing, swallowing and head balance, and how the training can accompany splint therapy.

Orthodontics

Mouth breathing, tongue thrust and an open lip posture are forces that act continuously on the teeth and jaws. You will find out how to record them before, during and after treatment and incorporate them into your treatment plan. More on this under Orthodontics and FaceFormer.

Speech and language therapy

Myofunctional disorders, swallowing patterns and articulation in connection with breathing and head balance. The exercises can be integrated into the treatment of children and adults.

Physiotherapy

Take recurring neck, back and head complaints into account along the control axis from the mouth to the pelvis. Head balance and pelvic floor are separate exercise modules.

Osteopathy

The CCFM describes the oral cavity, cervical spine and trunk as one shared control system. On top of that, you get a training programme that your patients continue on their own at home.

Myofunctional therapy

Instead of strengthening individual muscles in isolation, lip closure, nasal breathing, tongue alignment, swallowing and head balance are practised in the same sequence. The shape of the FaceFormer sets the direction.

The seminar is also aimed at doctors in ENT, sleep medicine and paediatrics and adolescent medicine, as well as coaches with a professional background. In Frankfurt, participants came from speech therapy, dentistry, physiotherapy, osteopathy, Heilpraktiker practice (non-medical practitioners) and other professions, some with many years of experience with the FaceFormer, many without.

Background

Why symptoms often return

Craniomandibular dysfunction (TMD), tooth migration, neck complaints, snoring, mouth breathing in children: different specialities, different diagnoses and, in our view, usually a common pattern behind them.

FaceFormer therapy does not treat an individual symptom. It trains the basic functions of breathing, swallowing and head balance. If their interplay deviates from the reference order, the body compensates, and the nervous system stores the compensation as a preferred pattern. In our view, an improvement in the complaint is a frequent consequence of the corrected function, not its actual subject.

  1. The pattern deviates

Mouth breathing, a low-lying tongue or disturbed head balance, often developed early, for example through dummies, bottles or habits.

  2. The body compensates

The nervous system automates the compensation. The basic programme is overlaid, not erased, and can be relearnt.

  3. Structural consequences

Palate shape, tooth position and posture adapt. Muscles that are not integrated into their function lose tone.

  4. Only then symptoms

When and how severely depends on the extent of the disturbance, the duration of the compensation and age.

Each of the three functions supports the others and is supported by them.Dr. Klaus-Jürgen Berndsen, “Trust the Triad” (in preparation)

Such complaints are often treated individually in different practices, while the common starting point is left unconsidered. What this looks like for individual topics is shown in our articles on TMD, the tongue reference position, swallowing and snoring and sleep apnoea. The fundamentals of the method can be found under How does FaceFormer therapy work?.

A patient has worn an occlusal splint for years. Her jaw complaints improve and then return. In terms of the CCFM, what would you look at first?

Course

How the training course works

Two days that build on each other: first understanding and assessment, then application and practice.

  1. Before the seminar

    Speciality and your own cases

After you register, we ask about your speciality, your areas of focus and cases from your practice. This allows the speakers to address them specifically.

  2. Day 1

    The model

The Cranio-Cervical Functional Model with the triad of breathing, swallowing and head balance, with external and internal systems, compensation and causal chains.

  3. Day 1

    Assessment and diagnostic scheme

Functional analysis and assessment from symptom to cause, the diagnostic scheme and the derivation of indications.

  4. Day 2

    Working with the FaceFormer

Exercise selection, dosage and correction. You carry out the exercises yourself, from the basic exercise to the advanced modules.

  5. Day 2

    Patient groups and cases

Indication-based application from children to adults, patient presentation, discussion of your cases and colloquium.

  6. After the seminar

    Materials and directory

Digital accompanying materials for reference, certificate, certificate of attendance and, if you wish, an entry in the practitioner directory.

The schedule is the same on both days: start at 9:00, coffee breaks at 10:30 and 15:00, lunch at 12:30, finish at 17:00. A training device and manual will be ready for you, so you do not need to bring anything.

Day one

Understanding and diagnosing

The first day is about the model behind the method and an assessment you can use as soon as your next day in practice.

The foundation is the Cranio-Cervical Functional Model (CCFM) developed at ISST Unna. It describes the oral cavity, pharynx, cervical spine and trunk not as the sum of individual findings from different disciplines, but as one shared control system. We treat it as a working model: it organises findings, and where its connections have not yet been independently tested, we say so.

From this follows a structured diagnostic scheme. You learn to assess breathing, swallowing and head balance in their interplay, to record orofacial findings such as tongue reference position, lip closure and masticatory function, and to establish links to posture and statics.

Functional analysis

Assessing breathing, swallowing and head balance in their interplay, first in the habitual state, then through targeted testing.

Orofacial findings

Tongue reference position, lip closure, masticatory function and swallowing pattern.

Posture and statics

Craniocervical connections extending to the trunk and pelvis.

Autonomic indicators

Sleep quality, recovery and resilience in everyday life.

Derivation

From assessment to an indication-based exercise programme.

The working basis is the assessment form, which you can also download without attending the seminar as a diagnosis form. In the seminar, you practise the tests on each other, because a functional assessment only becomes reliable through practice on real people. The most important rule: first observe unnoticed, then ask and test.

Day two

Applying and treating

On the second day, you work with the FaceFormer yourself. Anyone who knows the exercises from their own body can guide and correct them more precisely.

The focus is on structured exercise programmes: exercise selection, dosage and correction, as well as variants for different patient groups, from children to adults. You practise in the group and in pairs, while the speakers observe and correct.

In the patient presentation and colloquium, you discuss cases together, including those you have brought yourselves. This also includes the question of when the training is not indicated or should only accompany medical treatment.

Night-time use

Only as a supplement to daytime training, never as the sole passive support. To the module

You will receive the exercise videos from the seminar afterwards, as 3D animations and live-action film, for reference. The modules are also publicly available with instructions under FaceFormer training: Basics. You can refer your patients there.

The dosage follows a learning process. A compensation pattern is not abandoned through insight, but displaced through precise, frequent repetition. The training therefore runs three times a day for 6 to 12 months, followed by maintenance training.

Safety and evidence

Knowing the limits, assessing honestly

Part of applying the method is knowing when the training is not suitable or should only run alongside other treatment, and what the evidence supports.

Acute inflammation

in the mouth, throat or airways: let it subside first, then begin.

Completely blocked nose

have this checked by a doctor first before nasal breathing is trained.

Recent surgery in the mouth

Training only after clearance from the treating practice.

Acute jaw joint locking

with severe pain: have this checked by a doctor or dentist first.

Degenerative neuromuscular diseases

such as ALS, advanced Parkinson’s or bulbar palsy: only after consulting a doctor.

Obstructive sleep apnoea

only alongside sleep medicine treatment, never in its place.

Typical feedback at the start includes muscle fatigue, pressure on the teeth, irritation of the mucous membranes in long-standing mouth breathers, or a device that falls out at night. In that case, take a short break and continue at a lower intensity, with fewer repetitions or with the FaceFormer ZERO. Children under 4 years only train with expert supervision.

Medically prescribed treatments such as a splint, CPAP or nasal spray are never discontinued on one’s own initiative. The training can accompany such treatment; decisions about changes are made by the treating practice. How FaceFormer therapy fits alongside other approaches is explained under FaceFormer and other treatments.

We are also clear about the state of the research. The only randomised study is the Hamburg study with 45 children and adolescents; its endpoints are the basic functions1. In addition, there is a peer-reviewed study from Jena with a control group involving 187 people with muscle-related jaw pain2. The rest are pilot observations and posters. An overview with assessment can be found under Studies and science.

The absence of a study does not mean that the connection does not exist. It means that it has not been tested.

After the seminar

Certificate, materials and support

The work does not end with the second day of the seminar. You take these tools with you into your practice.

After the Frankfurt seminar, all participants received a digital folder: a practice guide with exercises, dosage, assessment and progress monitoring, a review of theory and diagnostics, 25 exercise videos, guidelines for working with patients and a collection of studies and sources.

In addition, there is the printed Manual on the methodology with the model, exercise modules, training plan and assessment form, as well as a FaceFormer training set for your own practice.

Speakers

From the developers of the method

The seminar is led personally by Dr. Klaus-Jürgen Berndsen and Sabine Berndsen. They developed FaceFormer therapy together in 1999.

Dr. Klaus-Jürgen Berndsen teaches the theoretical foundations: CCFM, functional analysis and diagnostics. He is a physicist, patholinguist and neuroscientist, was a lecturer at the University of Dortmund for many years and works in neurological rehabilitation. In 1984, together with Sabine Berndsen, he founded the ISST in Unna, which he has headed ever since.

Sabine Berndsen works as a therapist at the ISST herself and contributes the application from everyday practice, from working with children to complex cases. She designs the webinars on training with children and with adults together with Lena Koopmann.

Get started

Ready for the training?

The free FaceFormer app guides you through the basic exercises, counts along and reminds you of your sessions. If you wish, a practice from the practitioner directory can support you.

FaceFormer training aims to restore the physiological setting of the basic functions of breathing, swallowing and head balance, with lip closure and the tongue reference position, thereby creating the conditions under which symptoms can improve. It does not replace a medical assessment. You can find practitioners near you in the practitioner directory.
